ATM/Debit Card Request

Current members with active checking accounts who wish to request an ATM/Debit card or members with a only a savings account wishing to request an ATM card should complete the ATM/Debit Card request form.

This application should take you approximately 10 minutes to complete. You must be a U.S. citizen 18 years of age or older.

This form is intended for use by current members who wish to add services to an existing account. To add services online, you must acknowledge the Account Agreement & Disclosures at the bottom of the page. If you are not a member of Blue Flame Credit Union and wish to join, please use the Membership Application. If you are a current member requesting an ATM/Debit card and you do not have a checking account, you must complete the Add Accounts & Services form.

U.S.A. Patriot Act

In order to comply with the U.S.A. Patriot Act, Blue Flame Credit Union is required to verify the identity of the members applying for and opening new accounts/services or adding signatories to existing accounts/services with Blue Flame Credit Union. Information that we are required to obtain includes:

• Name
• Mailing Address
• Street Address (if different)
• Social Security Number or Tax Identification Number
• Date of Birth
• Copy of government issued photo ID (such as Driver’s License/State I.D, passport, Student ID, State issued ID, etc.)

Additional data or identification, as required under the U.S.A. Patriot Act, may also be gathered depending on the type of account applied for or opened.

Data on existing members will be gathered as they open or use additional services offered by Blue Flame Credit Union or add signatories to accounts.

The U.S.A. Patriot Act requires Blue Flame Credit Union to maintain records of the identification verification and update the information.

Confidentiality of this information will be maintained as required under the Privacy Act and all other applicable laws and regulations.

Prior to opening additional accounts or certificates, you must agree to our Account Agreement & Other Important Disclosures.
